Cytokine abnormalities in a patient with eosinophilic fasciitis.

چکیده
BACKGROUND Eosinophilic fasciitis (EF) and peripheral eosinophilia are rare in children. OBJECTIVE To describe a case of EF in a 3-year-old child who presented with acute painless induration of her forearm. METHODS Cytokine profiles were obtained and compared with patients with atopic disease during the acute presentation and after treatment with low-dose prednisone. RESULTS The patient's serum showed elevation of transforming growth factor beta1 and interleukin-5, which improved after treatment with low-dose prednisone. CONCLUSIONS The case suggests that patients with EF have cytokine abnormalities similar to atopic patients, but with a striking elevation of transforming growth factor beta1. The responsiveness of the clinical symptoms and cytokine profile to low dose prednisone supports treatment with low dose immunosuppressive therapy in this disorder.

Eosinophilic Fasciitis Associated with Autoimmune Thyroiditis
Eosinophilic fasciitis (EF) is scleroderma-like disease without Raynaud's phenomenon or visceral involvement. It is characterized by painful swelling of the extremities, accompanied by rapid weight gain, fever and myalgia. The acute state of disease is associated with significant peripheral blood eosinophilia, an elevated erythrocyte sedimentation rate and hypergammaglobulinemia. EF is also fre...
متن کاملCapillary microscopy in eosinophilic fasciitis. A comparison with systemic sclerosis.
Capillary microscopy was performed on 19 patients with eosinophilic fasciitis. These patients were compared with 13 individuals with progressive systemic sclerosis (scleroderma). Capillary patterns were normal in 16 of 19 (84%) eosinophilic fasciitis patients; 3 exhibited either borderline or nonspecific changes, and none showed a definite scleroderma pattern. In contrast, characteristic nailfo...
متن کاملTreatment with cimetidine of atypical fasciitis panniculitis syndrome.
Three patients presented with septal fasciitis and panniculitis, associated with clinical and laboratory features which precluded straight-forward classification into eosinophilic fasciitis, localised scleroderma, or lupus erythematosus profundus. Treatment with cimetidine caused the remission of cutaneous manifestations and the extracutaneous abnormalities, such as nailfold capillary disturban...
متن کاملSurgical treatment of lower limb ischemia and abdominal aorta aneurysm in a patient suffering from eosinophilic fasciitis.
We report on a 65-year-old woman with typical clinicopathologic features of eosinophilic fasciitis (Shulman syndrome) who suffered from lower limb ischemia and abdominal aorta aneurysm. Physical examination, laboratory data and histopathologic findings confirmed the diagnosis of eosinophilic fasciitis. The patient underwent aorto-bifemoral vascular prosthesis implantation. Tissue reactions asso...
متن کاملEosinophilic fasciitis in an African--possible benefit of chloroquine treatment.
A 58-year-old black woman with eosinophilic fasciitis is reported, this being the first report to our knowledge of the condition from Africa. The patient showed a subjective and objective improvement in symptoms on two occasions when treated with chloroquine.
